For All Appointments
New Patients: Arrive or log in 10–15 minutes early so we can complete your check-in without cutting into your visit time and wear comfortable clothing
Photo ID Driver’s license, passport, or other government-issued ID for verification.
Payment Method Cash, credit/debit card, HSA/FSA card, or check (payment due at time of service for cash-pay visits).
Current Medication List Include all prescription medications, over-the-counter drugs, vitamins, and supplements, with doses and how often you take them.
Allergy List Include drug allergies, food allergies, and reactions you’ve experienced.
Recent Health Records Any lab results, imaging reports, or hospital discharge papers from the last 6–12 months (especially important for new patients).
For Chronic Condition Visits (Diabetes, Hypertension, etc.)
Home Monitoring Logs
Blood sugar readings (write down or bring device)
Blood pressure records
Weight tracking
Specialty Reports Eye exams, foot care notes, or other specialist reports if available.
For Telemedicine Visits
Quiet, Private Space with good lighting
Device with Camera & Microphone (smartphone, tablet, or computer)
Charged Battery or plugged-in device
Stable Internet Connection
Any Documents or Photos Needed (lab results, rash photos, etc., emailed or uploaded beforehand if possible)
For Mobile/In-Home Visits
Clear Space for Examination (kitchen/dining table, desk, or counter)
Pets Secured for safety and focus during visit
Any Home Medical Devices you use (glucometer, BP cuff, inhalers, etc.) ready to review
Access to Medications so we can confirm your list
